Student Insurance (If student has no medical insurance)
Coverage Period: June 2026 – May 2027

Directions: All students interested in participating in athletics must first be cleared by following the steps below:
1. Complete a physical exam before starting the online registration process.
2. Complete the entire registration process on athleticclearance.com (link here).
a. You will need to upload your completed physical exam form.
*the form must be stamped, dated, and signed by your doctor.
*the Health History section must be completed.
*the form must have your name, date of birth, student ID #, grade, and sport(s).
*the physcial form must be signed and dated by your parent (seves as consent to participate in athletics and acknowledgement of all electronic signatures).
***Always keep the original copy of your physical exam for your records. If a coach
asks for your physical, then provide a copy of it, NOT the original!
b. You must provide your medical insurance information. Every student participating in athletics must be insured.
*If you do not have insurance, then you will need to purchase and provide proof of insurance before student will be allowed to participate in athletics.
*Note: If purchasing student insurance for Football Players, you must purchase one of the three policies found under “Optional Football Coverage”.

3. Students are not officially cleared until verification has been made that:
a. The physical exam is completely filled out and the doctor has granted full clearance, and
b. The entire online registration process has been completed.
Once verified, then student will be cleared to participate in athletics for the 2026-27 school year (or up to the expiration date of his/her physical exam – which expires 1 year to the day of the date listed on the physical form).
